Model State Forms Updated

The model state IEP forms have been updated to align with the new state assessment system that began in the 2014–15 school year and to address some housekeeping items. The revisions are described below:

IEP Form – without Secondary Transition (form 6c)

  • Pages 3 and 4 (Purpose statement) – removed references to the WAAS Portfolio
  • Pages 3 and 4 (Points to Consider) – removed “concurrent with the issuance of report cards” from the last bullet
  • Page 5 (Points to Consider) – removed the bullet related to the Certificate of Individual Achievement (CIA)
  • Page 5 (State Assessments) – revised to align with the new assessment system, removed references to the WAAS Portfolio

IEP Form – with Secondary Transition (form 6d)

  • Pages 5 and 6 (Purpose statement) – removed references to the WAAS Portfolio
  • Pages 5 and 6 (Points to Consider) – removed “concurrent with the issuance of report cards” from the last bullet
  • Page 7 (Points to Consider) – revised the fourth bullet to clarify that other assessment options are available “if required to meet graduation requirements”
  • Page 7 (State Assessments) – revised to align with the new assessment system, removed references to the WAAS Portfolio
  • Page 7 (Graduation) – moved graduation to its own section
